Surplus forms can be obtained -- HERE
How to correctly fill out Surplus forms
- Always include the Serial Number of the unit. Some units have a CAMs tag in addition to the s/n, make SURE to add these. CAMs tags are just as, if not more important, than serial numbers.
- Mark all items with a number that corresponds to the number you assign it to on the surplus form. You will have to actually assign each item to a number on the surplus form, and then write that number on the item.
- Remove or erase ALL hard drives from machines. For servers, check for multiple drives. Information for hard drives removal can be found --HERE
